The Dallas Day Camp Health Form is required. Every camper must have a current and signed Health Form on file before he/she will be admitted to camp. Please download and bring with you on the first day of camp. Click here to download the Health Form.

Release Forms - You will also need to bring a completed Transportation Release Form and a Rock Wall Climbing Release Form.

The Camper Tee and Sky Packs Order Form allows you to pre-order the "official" Camper Tee of 2010,  Sky Pack gift packages and the Camper Photo.  This form will be available at check-in.
